To get started:
- Place your order
- Complete appropriate chain of custody PDF (anonymous or not)
- Email completed PDF to themushroomchamp@gmail.com with your order number as the subject.
- Send in 1.25-1.5g sample in a container of your choice (ie. plastic bag, glass jar/lid, etc) WITH dessicant pouch to minimize moisture aborption
- Package sample with proper padding, Order Number, and send to:

If anonymous, we will forward your test results via email, otherwise you will recieve an email from Altitude Consulting with results.
